Graham County Community Vision
We seek to create and maintain an environment that is clean, safe, and healthy and an educated community in which all individuals can achieve their optimum physical, cultural, social, economic, mental and spiritual well-being today, tomorrow and in the future.

Health in Arizona Policy Initiative (HAPI)
The Arizona Department of Health Services (ADHS) is partnering with local county health departments to improve health outcomes for Arizonans. Health in Arizona Policy Initiative (HAPI) will focus on achieving targeted improvements in health outcomes by advancing evidence-based strategies which address community design, school health, worksite wellness, clinical care, and accessibility to nutritious food. The program is funded with federal Title V and state tobacco tax & lottery prevention funds.

The HAPI grant will address community and health concerns, and help develop policy change to improve the quality of life in Graham County.

Community Health Assessment (CHA)
The Community Health Assessment (CHA) is funded through Arizona Dept. of Health Services (ADHS). The purpose of the CHA is to learn about the health status of Arizona’s population.

What do we do? Through collaboration and partnerships, the evaluation and dissemination of information that is gathered from facilitating community meetings, conducting the Community Assessment Survey, creating and implementing plans to improve the health of the citizens of Graham County, this grant funded program is the Voice of the Community concerning health related topics.
