Graham County Health and Community Health Worker Readiness Survey

We are beginning a strategic planning process towards implementing a pilot Community Health Worker program. An important part of the planning is to understand more about the community in which the program will operate, so we are interviewing members of the community to understand  what kinds of health issues people in Graham County face and to inform people about the type of work Community Health Workers do. Your organization (whatever it is: food pantry, schools, library, etc) is a valuable part of the community and we would love your help in spreading awareness and increasing participation in our surveys in order to reach as many people in the county as possible. 

This survey is part of an assessment by the Graham County Health Department in collaboration with the Southeast Arizona Health Education Center (SEAHEC) and University of Arizona College of Public Health to learn about Graham County health priorities and how a Community Health Worker program might benefit its residents. 

  • Your responses will be anonymous and we appreciate your voluntary participation in this survey. 
  • The survey takes 5 minutes at most to take.  
  • The results will be shared with all county residents by the Graham County Health Department.
